Appointment Request

Use this form to request an appointment with us for non urgent appointments. If during office hours, you will be better served by calling us directly at 905-567-8387 to make an appointment.  While we will do our best to accommodate your requested day and time, please note, your appointment is not fully booked until you get a confirmation from us!

PLEASE NOTE: THERE IS A $35 RESERVATION FEE FOR ALL NEW CLIENT/PATIENT APPOINTMENTS.  This payment may be made over the phone with a CreditCard or by e-Transfer.  This deposit is applied towards your visit or forfeited without 24h notice of cancellation/rescheduling.

Please do not assume you have an appointment unless you get a confirmation from us.
